Mulkey was one of the first girls in the U.S. to play organized basketball with boys and led her Hammond High School basketball team to four consecutive state championships. She was an All-American point guard at Louisiana Tech University, winning two national championships as a player and the inaugural winner of the women's version of the Frances Pomeroy Naismith Award.

In 1984, she represented the U.S. at the 1984 World Championships, where the team won six games but lost two against the Soviet Union. In 2000, Mulkey took over a Baylor program that had finished the 1999–2000 season 7–20 and last in the Big 12 Conference. The Lady Bears program won its first NCAA tournament bid in 2005, making her the first woman to win NCAA Division I basketball titles as a player and head coach.

Mulkey has been involved in the NCAA women's tournament every year except 1985 and 2003. In 2007, she signed a 10-year extension to remain Baylor's coach. In 2012, Mulkey made NCAA history by leading the Lady Bears to a perfect 40–0 season, the most wins in college basketball history, men or women.

During the COVID-19 pandemic, Mulkey advocated ending COVID-19 testing on tournament players despite the ongoing pandemic. She was announced as head coach at LSU on April 25, 2021, and won the NCAA National Championship for the first time as coach of LSU in 2023.
